The relative of a signer called in on the afternoon of November 3, 2015, for a patient staying at the Regency Hospital, residing on the Fourth Floor of the St. Francis Bon Secours Downtown facility. Due to scheduling conflicts on behalf of the lawyer and notary, the signing did not take place until the morning of November 4, 2015.
